Any idea what is going wrong here?

We have abandoned the HeaderFooter extension now and switched to PageNotice, which offers pretty much the same capabilities.

I'm sorry, but why is this not mentioned on the Extension page and why is PageNotice still referring to HeaderFooter as if it was a sensible alternative?
