"A form is made in HTML which store the input in MySQL database using PHP": what does this even mean? Do you plan to do this in MediaWiki, or on a separate website? The synopsis says "The goal is to either create or adapt a free and open source review system", did you consider any existing application or any MediaWiki extension?
