When trying to rename a user I get the following error. Any suggestions?

Please also state the versions used for PHP and MediaWiki to make it easier for developers to pin the issue.

I'm also getting the following when trying to rename a user account (the extension used to work... we've been installing additional extensions and now it doesn't).
