Right, seeing all the unanswered threads in this talk page, I know there's about a 99.99% chance I'll go ignored like everyone else, but here goes.
The page shows up okay for logged in users and auto-fills their name (or username if no name) and their e-mail. Running MediaWiki 1.26.
Also, it doesn't actually send the e-mail.
