I am having trouble to install this extension, well sort of. Using <timeline> timeline syntax </timeline> works like charm. If I use {{#tag:timeline| timeline syntax }} the text is not showing up. This advice seems unrelated and does not help anyway.
The wiki is on MW 1.22.5 and PHP 5.3.3 with the latest pre json-i18n code.
I will very much appreciate any hint on how to resolve this.
Seven months have passed and things have happened in the meantime. After upgrading to Debian 7 and MediaWiki 1.23 even the tag stopped working, so no tag and no parser function. :( However I got it working now. The issue was that I did not add $fontFile = 'ascii'; to the "LocalSettings.php" file after invoking the extension. After I have done this everything was fluffy. A short note about using the parser function: There must be a blank line between the function call and the timeline syntax, e.g.

timeline syntax ...
}}
I had this problem too. Tag doesn't display any text (I didn't use the parser function). I used the sample code from the page. I've already add $fontFile = 'ascii' to no avail.
Phab: T18085 does not seem to address this issue for general (non-Wikimedia wikis) use.

Hmm, if $fontFile = 'ascii'; does not work perhaps ploticus is not installed correctly or in a different spot. Perhaps you have to adjust $wgTimelineSettings->ploticusCommand = "/usr/bin/ploticus"; too.
Turns out a cache problem. First I installed FreeSans font, and added the location to the LocalSettings.php putenv("GDFONTPATH=/usr/share/fonts/truetype/freefont"); but then I didn't clear the cache, I only previewed the edit from the editor window. What I did next to solve the problem was I saved the page that I was editing, deleted the generated images from "w/images/timeline" folder, and hard refresh the page.
