Namespace prefix o on p is not defined

A few pages on our media wiki have started providing the below error message, but we are not certain why or what is causing it, looks like a database error to me.

It only appears on a few pages, and similar pages still works.

It's trying to parse content that is invalid. Either the content is invalid, or you have some extension installed that is injecting something invalid.

How do I get to the offending site, considering that I only get the error message...

For anyone who gets the same issue, there may be a simple work around (worked for me at least). Just roll back to earlier version of the document.
