One of the original uses of this template is here Manual:Short_URL. I am sure there are alot of other similar uses of this same template, for implementation of worked/didnt work.

Purpose:
This template should be used when a topic/category has been identified and the implementors of the topic/category have noted that the potential for problems in various different implementations are aparent. As such, they need feed back to see if the suggested solution was helpful or actually worked in a specific environment.

It is suggested that anyone that utilizes this template also make sure they have an appropriate feedback discussion (talk) available for use by the users.

Disclaimer
As this template is dependent on user's incrementing the value up on either parameter (worked & didn't_work) this should not be used as bonifide guage of solutions viability. As well, you may find that the dissatisfied users of your Guide may be more quick to increment the didn't_work more so than the users that implemented your solution sucessfully.

This is only a subjective scoring mechanism and should not be used a a rule of thumb.
